Medication Refill

 

  • Allergy

  • Asthma

  • Diabetes

  • Depression (does not include sedatives such as Xanax or Ativan)

  • Gout

  • NSAIDs 

  • High Cholesterol

  • Hypertension

  • Hypothyroidism

  • Oral Birth Control

 

  • Medication prescription has been written by another provider within the past 12 months

  • Medication prescription has been expired no more than 30 days 

  • In order to prescribe, we need the following information:

    • the name of the medication​

    • the dosage

    • the frequency (how many pills per day and how many times per day)

  • Our refills includes a 90 day supply of medication(s). 

  • We will allow an emergency visit, for a 30 day supply of medication, after initial 90 day supply is complete.

  • A total of 120 days / 4 months of medication will be allowed per calendar year.

  • We do not change current medication dosages, nor, do we initiate new medication(s).

  • are experiencing side effects or withdrawals

  • desire a change in medication dosage

  • desire to switch to a different medication

  • desire to stop medication

  • under the age of 18
